Patients

As soon as you arrive, you will receive a friendly greeting from our new patient coordinator. Enjoy a cup of tea or bottle of water at check in and if you have any questions, please ask. Plan to spend about an hour with us during a complimentary consultation.
After we update your records and take digital X-rays or Panorex images, Dr. Kelly will talk with you to get your background and learn about your expectations as a new patient. Once the doctor finishes his analysis, we will begin planning the treatment phase, one step at a time. If your smile goals require multiple treatments your doctor will work with you to form the optimal treatment plan. Together, you and your dentist will create a solution that addresses your needs and creates your ideal smile!
To expedite the registration process, complete these forms prior to your visit and bring them to your appointment. Please contact us if you have any difficulty accessing the forms below.

Post Operative Instructions

Post-operative care is important following oral surgery and recovery may be delayed if this care is neglected. Some swelling, stiffness, oozing of blood and discomfort is expected after surgery. It is helpful to have the patient observed by a responsible adult for the duration of the day of the surgery. The following includes our post-operative instructions and events, which may take place following this kind of surgery.

Insurance and Financing

We don’t want financial issues to stop you from getting the treatment you need. For your convenience, we also accept payment in the form of cash, check, or credit card, including American Express, Discover, MasterCard, and Visa. Third party financing is available through Care Credit and Lending Club for qualified applicants. If you have any questions about insurance, don’t hesitate to call us.
